Community Partners
Our Community Healthcare Partners
Community Healthcare Services are the broad range of services that are provided outside of the acute hospital system and includes Primary Care, Social Care, Mental Health and Health & Wellbeing Services. These services are delivered through the HSE and its funded agencies to people in local communities, as close as possible to people’s homes. Community Healthcare services focus on keeping people well so that they can continue to live at home or close to home through services such as health promotion, disease screening, diagnosis, treatment and rehabilitation programmes.
