Clinical Placement Students can use this Form to:
- report a health issue affecting their placement
- request a medical certificate
- upload immunisation records or
- request a copy of immunisation records

Student Health staff review these Forms 9:30 am- 12:15 pm Monday to Friday.
If you have notified us that you have been unable to attend placement for medical reasons we will call you on either the day of notification or by 12:30pm on the next working day.
If you have requested your immunisation records we will let you know what is happening via SMS to your mobile numbers within 7 working days.
Do not use this Form if you need urgent help.
