How to access your Student Health Medical Record

You are entitled to access and obtain a complete copy of your Medical Records or a portion of the record/Medical Data on your Record, either to collect in person or to request that the record be posted or e-mailed securely to your GP, or another Doctor or Medical Service. It is vital to protect the confidentiality of what is sensitive personal information to ensure that your medical records are requested only by you and/or your medical treating GP, Family Doctor or Medical Specialist.

REQUEST COMPLETE RECORDS FOR COLLECTION IN PERSON

If you wish, a hardcopy of your complete medical records can be made available to you for collection by you in person. You will need to download, complete and forward to the Student Health Deparment the following Collect Records request form. Collectrecords Please note you will need to provide photo-ID when calling to collect the hard copy of the records.  If you wish to nominate a third party to collect medical records on your behalf data you will need to nominate them in writing and they will need to provide Photo ID to confirm their identity on collection. You will also need to complete the Collectrecords form.

 

REQUEST COMPLETE RECORDS FOR DELIVERY VIA REGISTERED POST

If you wish, a hardcopy of your complete medical records can be made available to you for delivery via Registered Post. You will need to download, complete and forward to the Student Health Deparment the following Post Hard Copy Records posthardcopyrecord request form. Please note you will need to include a scan copy of photo-id such as your Student Card with your request.

 

REQUEST COMPLETE RECORDS BE TRANSFERRED TO YOUR GP/OTHER DOCTOR VIA REGISTERED POST

If you wish, a copy of your complete medical records can be transferred to your  GP or Family Doctor or other Medical Doctor. You will need to download and complete the following transfer records request formtransferrecords and ask your Doctor to complete the form too, before returning it to the Student Health Department. Please note you will need to include a scan copy of photo-id such as your Student Card with your request. Use this option if you wish a complete copy of your medical record to be forwarded to another clinician on your behalf. 

 

REQUEST A PORTION OF YOUR RECORDS FOR  COLLECTION IN PERSON 

If you wish, a hardcopy of a portion of your medical records can be made available to you for collection in person . You will need to download, complete and bring with you in person the appropriate  accessportionmedrecordinperson request form. Please note you will need to include a scan copy of photo-id such as your Student Card with your request. If you wish to nominate a third party to collect medical records or data on your behalf, you will need to nominate them in writing and they will need to provide Photo ID to confirm their identity on collection.  Use this option if you wish a copy of blood test results or immunisation history or other similar subset of your record. Please provide details of what is required on the Form.

If you wish, a hardcopy of a portion of your medical records can be transferred to your  GP or Family Doctor or other Medical Doctor via registered post or by secure e-mail.  You will need to download and complete the following Transfer Hard Copy Data Request Form, transfermeddata and ask your GP to complete the form too, before returning it to the Student Health Department. Please note you will need to include a scan copy of photo-id such as your Student Card with your request.  Use this option if you wish a copy of blood test results or immunisation history or other similar subset of your record to be forwarded to another clinicain on your behalf.
