You can use the Form below to request an electronic copy of your medical record be sent to you via an encrypted email from UCC Student Health.

You can also use the Form below to upload and send health documents to us securely.

Tell us on the Form what you want:

• to have a soft copy of your record sent to your  email address OR
• to have a soft copy of your record sent  to your own Doctor's email address OR
• to upload your health documents to us securely

Student Health staff review these Forms 9:30 am- 12:15 pm Monday to Friday only.

You will need to be logged on using your UCC Student email ID to use the Form. You may need to adjust your Cookie settings to use the Form

IF YOU NO LONGER HAVE A UCC LOG-ON or YOU ARE UNABLE TO USE THE FORM

If you are no longer a student and/or cannot log onto to the secure FORM above, the following are your options:

REQUEST YOUR RECORDS BE SENT TO YOU AT YOUR NON-UCC EMAIL ADDRESS

A copy of your medical records can be sent to you at your non-UCC email address via encrypted email
