Notification of Maternal Deaths

MDE Ireland aims to report on all cases of maternal death occurring during or within one year of the pregnancy.

In the event of a maternal death occurring during or within one year of the pregnancy, please contact the Maternal Death Enquiry (MDE) Ireland office by phone or by email. We will record the details of the case and arrange for the relevant paperwork to be sent to you.

A dedicated MDE Maternal Death Notification Form is available from the MDE Ireland office / and this web site. In all maternity units, a unit co-ordinator has been identified to liaise with the MDE Ireland co-ordinator. Cases can also be notified to the MDE office by acute general units, coroners, general practitioners and public health nurses.
