• Reduce the number of women having a MOH/LTH
• Reduce the number of women requiring ICU level 3 care due to PPH
• To reduce the morbidity associated with PPH in ROI in 3 years:
• Reduce the number of women requiring ≥ 5 units of red blood cells
• Achieve a reduction in Fresh Frozen Plasma (FFP) Coagulation transfusion
• Ensure PPH/MOH are discussed at risk management meetings
• Encourage staff and patient debriefing following a PPH/MOH
• Reduce the women receiving hysterectomy due to PPH

The Process

• Collate an overview of all the clinical practices related to PPH in all 19 units with a view to examining what is working in some units and to provide an opportunity for shared learning.
• Collect data on all PPH/MOH cases
• Move all units to a standard process of measurement of blood loss after delivery
• Develop a staged approach checklist/standardised proforma for the management of PPH
• Update national guidelines to include definition update and checklist
