IF YOU NO LONGER HAVE AN ACTIVE UCC Email
If you are no longer have an active UCC student email address the following are your options:
REQUEST YOUR RECORDS BE SENT TO YOU AT YOUR NON-UCC EMAIL ADDRESS
A copy of your medical records can be sent to you at your non-UCC email address via encrypted email
Click emailrecords to download the Form and then mail the completed Form to us at
