Access your Health Record or Upload Documents to Us

You can access and obtain a full copy of your medical record (or a portion of your record) from Student Health.

You can also use the Form below to upload health documents etc. to us securely.

Tell us on the Form what you want:

• to be provided with a hard copy of your record for you to collect in person OR
• to have a hard copy of your record posted to you at your own address OR
• to have a hard copy of your record posted to your Doctor OR
• to have a soft copy of your record sent via Healthmail e-mail to your Doctor (secure Healthmail only*) OR
• to upload and send your health documents to UCC Student Health securely

Student Health staff review these Forms 9:30 am- 12:15 pm Monday to Friday only.

You will need to be logged on as using you UCC Student email ID to use the Form. 

*Records can only be sent by e-mail to your GP or another Doctor or Medical Service via the Healthmailservice.

IF YOU NO LONGER HAVE A UCC LOG-ON or YOU ARE UNABLE TO USE THE FORM

If you are no longer a student and/or cannot log onto to the secure FORM above, the following are your options:

REQUEST YOUR RECORDS BE DELIVERED VIA REGISTERED POST TO YOUR DOCTOR

A hardcopy of your medical records can be sent to your Doctor via Registered Post.

If you wish to collect your records in person yourself download this Collectrecords Form - please bring photo-id with you when you attend to collect your record

NOMINATING A THRID-PARTY TO COLLECT YOUR RECORDS IN-PERSON.

If you wish to nominate a third party to collect medical records or data on your behalf, you will need to nominate them in writing and they will need to provide Photo ID to confirm their identity on collection.

Please complete the Collectrecords Form and ensure they bring a co[py with them along with their own photo-ID and a letter of autorisation from you.
