FaME Implementation Toolkit (UK)
Please register and check the website www.fametoolkit.org.uk for the UK FaME Implementation Toolkit for a full suite of planning materials prepared for use in the NHS, but which could be adapted for use in Ireland to support the commissioning, design and delivery of FaME classes. CLINICAL COMMUNICATIONS, ASSESSMENTS AND REFERRALS
Developing a standard set of assessment tools and referral forms and processes is essential to streamline service provision and communication between healthcare professionals and community based exercise instructors. It is also good practice to build on existing referral pathways and tools, such as local ICPOP pathways or Healthlink.
