Integrating care between the acute hospital and the community is vital to the success of the Cork- Integrated Dementia Care across Settings IDEAS project.
An integrated care pathway for the person with dementia is currently in development to promote coordinated care from community to hospital and back to community. This will minimise delays, improve communication, and support patients and families during hospital admission and discharge. This initiative is led by Mary Mannix (acute/hospital component) and Siobhan Cahill Community Dementia Care Coordinator (community component).
The ICP has been piloted for one ward (Autumn 2016), and it is planned to roll out the ICP in other ward locations shortly.
