Ethical Approval Forms

A proposal for ethical approval must include the following four documents. These documents should be sent to:

You may send electronic copy by email in pdf format (preferred) or on a CD.

Documents required:

• A summary of the research proposal (max: 500 words);
• A completed Ethical Approval Form; Forms are available in these formats:
  • rtf (word processor) version of approval form.
  • HTML (web) version of approval form.
  • Excel (spreadsheet) version of approval form.
  • pdf (print) version of approval form.
• An informed consent form (see as example Informed Consent Form for Participants);
• A briefing for participants (see as example Model for Briefing on Informed Consent for Participants.)
