Dementia is under diagnosed but affects almost 30% of older people admitted to acute hospitals. As the population ages, this percentage will continue to rise. The Cork-IDEAS project is about improving care for the person with dementia in the Mercy University Hospital (MUH) and community. Integrating care between the hospital and community is a vital component of the project. MUH and key community stakeholders will work together to provide an integrated service for patients and their families, providing excellent acute hospital care where necessary, alternatives to admission where possible, and support for those caring for the person at home.
