Over the past five years , the need for doctors to learn and practice peripheral nerve blockade has been become glaringly and globally obvious. Cork has contributed a best selling textbook and CD rom, an internationally acclaimed “hands on” annual workshop (Depts of Anaesthesia and Anatomy, UCC) and numerous journal articles. High resolution medical ultrasound facilitates nerve, needle and injectate visualisation. It is possible to place a needle under real-time ultrasound guidance adjacent to the brachial plexus, and watch as local anaesthetic injectate surrounds the plexus. This technique has resulted in an improvement in the precision of perineural needle placement and a reduction in the dose of local anaesthetic required to perform successful brachial plexus anaesthesia. Attached are the lead article in Anesthesiology (July 09) by Brian O’Donnell (SIVUH and Honorary Senior Lecturer UCC) and Gabriella Iohom (CUH and Senior Lecturer UCC) and the accompanying editorial. The article describes the utility of ultrasound-guidance in reducing the dose of local anaesthetic required for axillary brachial plexus block in patients undergoing elective upper limb surgery.
